Health insurance and medical cannabis in 2026

For most patients, the question of cost coverage is the central concern of any cannabis-based therapy. Since the liberalisation of October 2022 the situation has shifted, but it is not yet uniformly regulated.

When the basic insurance pays

Mandatory health insurance (OKP) only covers the cost when a coverage guarantee has been approved by the medical adviser of the respective insurer. Conditions are generally:

  • A severe, chronic illness with documented suffering
  • Standard therapies already exhausted with insufficient results
  • A prescription by a physician experienced in cannabis medicine

Coverage request

The request is filed by the treating physician directly with the insurer. The following points help:

  1. A clear diagnosis with the history of the illness
  2. A list of treatments already attempted and their effects
  3. The expected therapeutic benefit of the cannabis therapy
  4. A dosage plan and an estimate of monthly costs

Patient’s share

Even with an approved coverage guarantee, a remaining share is often borne by the patient, depending on deductible and co-payment. If the request is refused, the costs are paid entirely out of pocket.
